What We Collect & What We Do Not
Plain-English privacy policy for vitaehealth.org and the Vitae Catholica telemedicine clinic. For patients, we follow HIPAA and Arizona / Wisconsin health-privacy law. For visitors, we keep the website's collection small, specific, and stated.

1. Who We Are
Vitae Catholica is an Arizona nonstock nonprofit corporation . This policy governs the website vitaehealth.org, the Vitae Catholica telemedicine clinic, and any email correspondence with us.
2. Website Visitors
What we collect
- Forms you submit — contact form, consultation request, newsletter subscription, waitlist reservation. We store what you give us (name, email, notes) so we can reply.
- Server logs — IP address, browser type, pages visited, timestamps. Retained for up to 90 days for security and troubleshooting.
- Cookies — a small number, described below.
What we do not collect
- No tracking pixels from third-party advertising networks.
- No fingerprinting.
- No clandestine session replay.
- No selling of visitor data — ever, for any price.
3. Patients & HIPAA
If you book a clinical visit, Vitae Catholica becomes a HIPAA covered entity with respect to your Protected Health Information (PHI). We protect PHI under the HIPAA Privacy and Security Rules, 45 CFR Parts 160 and 164, and under applicable Arizona and Wisconsin medical-privacy statutes.
How we use PHI
- To provide clinical care, write clinical documentation, and communicate with other providers treating you.
- To bill for services, including submitting ESA or school-choice documentation when you request it.
- For operational tasks necessary to run the clinic (scheduling, secure messaging, quality review).
How we do not use PHI
- We do not sell PHI.
- We do not share PHI with marketing platforms.
- We do not disclose PHI without your authorization except as required by law (public-health reporting, court order, protection of life).
A Notice of Privacy Practices (NPP) specific to the clinic is delivered at your first visit and available on request.
4. Cookies
We use a small number of cookies, all strictly necessary:
- Session cookies — keep you logged in during a visit; expire when you close the browser.
- Preference cookies — remember if you have dismissed the cookie banner or accepted the privacy notice.
- No advertising cookies. We do not use Google Ads retargeting, Facebook Pixel, or similar.
You can disable cookies in your browser without breaking the site.
5. Your Rights
Because we collect so little, the list is short. But you always have the right to:
- Know what we have collected about you.
- Correct inaccurate information.
- Delete information we are not required to retain.
- Unsubscribe from any mailing with one click.
- As a patient, you also have the HIPAA rights to access, amend, and receive an accounting of disclosures of your PHI.
6. Changes to This Policy
If we change this policy materially, we will post the new version with a new “Last updated” date and, for patients, notify you. Minor edits (typos, clarifications) do not trigger renotification.
